Job Location: Lake Havasu City, Arizona
Job Summary: Hospice of Havasu, one of the largest and most successful not-for-profit hospice care agencies in Arizona, is seeking an Executive Director.
Organized in 1982, the main office is in Lake Havasu City, with staff offices in Parker and Bullhead City. Hospice of Havasu owns and operates the six-patient Polidori House and owns and operates the Hospice of Havasu Resale Store.
The Executive Director has over-all responsibility for the entire agency and business entities of Hospice of Havasu.
The Executive Director maintains the agency's hospice mission, and provides leadership by working with the Board of Directors and Executive Management Team to establish goals, strategies, plans and policies. The Executive Director will be visible in community events and participate with other agencies in the community and with hospice organizations at the state level.
The ideal candidate for the position will:
- Have extensive business experience and financial management experience, including Board relations, customer relations, community relations, fund raising, healthcare management, personnel management and program management.
- Have an MS degree in Human Services or Administration and at least 3-5 years of Administration or supervisory experience in community health programs.
- Have excellent written and verbal communications skills and skills in public speaking and presentations and fund raising activities.

Social Worker
Job Location: Lake Havasu City, Arizona
Position Status: Full Time, Part-Time; At Will
Job Summary: Responsible for the delivery of varied social work services to patients and
families to ensure that their emotional and social needs are met. Provide psychosocial assessments, ongoing
psychosocial counseling and education. Active member of the Interdisciplinary Team (IDT) assisting with
development and implementation of patient care plans.
Knowledge:
- Hospice philosophy of care.
- Human behavior and the procedures for assessment of psychosocial issues.
- Group behavior and dynamics; societal trends and influences.
- Principles and methods of teaching and instruction for individuals and groups.
- Computer literate in Windows environment; MS Office.
Skills:
- Develops comprehensive psychosocial assessments and reassessments for patients and families.
- Makes referrals to appropriate community resources; provide assistance in accessing services
(i.e. personal, business, financial, legal, housing, etc.)
- Counsels patients and families in individual and group settings to assist them in understanding and
coping with end-of-life issues.
- Provides short-term crisis intervention when indicated.
- Advocates for the best interests of patients and families.
- Completes clear, concise documentation in a timely manner.
- Assumes responsibility for own professional development, meeting the agency requirements for continuing
education.
Education: Bachelor’s or Master’s degree in Social Work from an institution accredited by the
Council on Social Work Education.
Experience: Two years of clinical work experience in a healthcare setting preferred.
Hospice Visit Nurse RN/LPN
Job Location: Lake Havasu City and Bullhead City (Area), Arizona
Position Status: Full Time, Part-Time; At Will
Job Summary: AZ licensed RNs and LPNs are needed to provide on-call and routine nursing care to
patients to maximize the comfort and health of terminally ill patients and their families.
Knowledge: Information and techniques needed to assess human injuries and diseases. This
includes symptoms, treatment alternatives, drug properties and interactions
Skills:
- Provides assessments and nursing care at the patient’s residence or facility setting; administers
treatments and medication in accordance with accepted standards.
- Assists in the development, monitoring and updating of individualized care plans.
- Completes records in compliance with agency policy and state/federal regulations.
- Communicates changes in patient’s condition to on call Hospice staff, Case Manger, physician or
other members of the health care team.
- Instructs patients/family units regarding basic nursing care, medication compliance and items
determined by IDT and scope of practice.
- Attends and participates in all appropriate meetings, training and development opportunities.
- Provides support to patient/family/significant other(s) to help meet the patient’s social and emotional needs.
Education: Current Arizona RN or LPN license, Current CPR certification.
Experience: One year of varied clinical nursing experience; home health or hospice preferred.
Certified Nursing Assistant
Job Location: Lake Havasu City and Bullhead City, Arizona
Position Status: Full Time, Part-Time; At Will
Job Summary: Provide basic patient care under direction of nursing staff. All care is
designated in the patient’s plan of care, as determined by the Interdisciplinary team.
Knowledge:
- Hospice philosophy of care.
- Principles and processes for providing excellent customer and personal services.
- Personal responsibility for representing the agency to the public and other external sources.
Skills:
- Provides personal care and simple, clearly defined nursing measures defined as assistance with
bathing, dressing, grooming, eating, ambulating or toileting to terminally ill patients under
the supervision of a registered nurse.
- Provides patients with help moving in and out of beds, baths, wheelchairs.
- Turns and re-positions bedridden patients, alone or with assistance.
- Feeds patients who are unable to feed themselves.
- Performs tasks and provides assistance per care plan to create and maintain a safe environment
for the patient; routine homemaker services defined as food preparation, cleaning, laundry,
changing bed linens and housekeeping.
- Provides support to patient/family/significant other(s) to help meet the patient’s social and
emotional needs.
- Maintains timely records of patient care, condition, progress, and problems in order to report
and discuss observations with case manager in accordance with policy and applicable regulations.
- Attends the Interdisciplinary team meeting and continuing education, as requested by supervisor.
- Maintains appropriate, current licensure/certification.
Education:
- High School diploma or GED certificate.
- Current Arizona CNA certification.
- Current CPR certification.
Experience: Two years professional CNA experience; less considered, if hospice specific.
