The Planning for the Future Workshop is open to the community. The workshop will cover Advance Care Planning and “What My Family Needs to Know”, which helps in organizing personal accounts and information.
Advance Care Planning is important for everyone, regardless of age. Letting others know of one’s
healthcare preferences is essential in the event of incapacitation resulting from illness or injury.
This workshop provides and reviews the Arizona Advance Directive Packet and discusses other
important conversations about “What to do When…” Advance Directives include:
• Health Care Power of Attorney
• Mental Health Care Power of Attorney
• Living Will
• DNR (Do Not Resuscitate)
• POLST (Physician Orders for Life Sustaining Treatment)
The “What My Family Needs to Know” workshop reviews and provides a workbook to help in identifying and organizing important documents and accounts.
